Plain-language privacy.

When you submit the school inquiry form, we collect the information you provide—such as your name, school, email, phone number, program interests, and event details—so the appropriate local FIIZ Drinks, everbowl, or Larson Family Orthodontics team can respond and coordinate your request.

How the information is used

• To answer your inquiry and discuss the school program you requested.
• To route your request to the appropriate local team.
• To plan or coordinate a program if you choose to move forward.

We do not ask for medical records, payment card details, passwords, or other sensitive information through this form. Please do not include those items in your message.

Form processing

The website is hosted by Cloudflare Pages, and form submissions are processed and stored by Formspree so email notifications can reach the appropriate local teams. Access is limited to people responsible for responding to school inquiries and operating the website.

Retention and deletion

Formspree’s Free plan keeps submission history for 30 days. Notification emails and any information retained for follow-up remain in the appropriate local team’s email system until the team determines they are no longer needed. You may request deletion of your inquiry and contact information at any time using the addresses below, subject to any recordkeeping required by law.
